Well-Child Check Chart

This chart was created for Lawndale Community Health Center. It provides a detailed checklist for new mothers to know when to take their child to their physician, and what immunizations their child needs. It was created as a magnet to be an easy reference in homes. The bottom of the chart includes the locations and phone numbers of the different health centers.
